How you can report a problem in healthcare safety or quality

You can help prevent future harm! If you experience or see a New York healthcare provider behaving in a neglectful, irresponsible or harmful way, please report it! Here are some helpful hotlines (all are located at the NYS Department of Health unless otherwise stated):

NYS Physician Complaint Line (to complain about a physician, physician assistant or specialist assistant): 1-800-663-6114

Office of Professional Discipline Complaint Line (to complain about a chiropractor, dentist, nurse, podiatrist, psychologist or other New York State licensed healthcare provider – NYS Education Department): 1-800-442-8106, or e-mail conduct@mail.nysed.gov or write to the NYS Education Department.

Hospital Patient Care Complaints: 1-800-804-5447

Nursing Home Complaint Hotline: 1-888-201-4563 or fax to 518-408-1157, or file a complaint online.

Adult Home Complaint Hotline: 1-866-893-6772

Home Care Agency or Hospice Program Complaint Hotline: 1-800-628-5972

Long Term Care Ombudsman Hotline (NYS Office for the Aging) 1-800-342-9871, or contact the Ombudsman Program online.

Managed Care Complaint Hotline (for both private and Medicaid coverage):  1-800-206-8125

Medicaid Fraud Hotline (NYS Medicaid Inspector General): 1-877-87FRAUD (1-877-873-7283)

Laboratory Complaints: 1-800-682-6056
